Service Agreement Template

This is a service agreement template GX Web Studio provides for our contractor clients to use with their customers. The template covers scope of work, pricing, change orders, warranty, cancellation, and dispute resolution. Bracketed fields (business name, addresses, project details, pricing) are filled in by the contractor before each customer signs.
Section 5: SMS Communications Consent
The template includes a Section 5 SMS Communications Consent clause that satisfies A2P 10DLC consent requirements. When a customer signs the agreement with that section checked, the contractor has documented written consent for SMS messages sent on the contractor's behalf through GX Web Studio.
